PATIENT FORMS

Patients that have been referred for a PET scan from an authorized physician will be contacted by CareImaging LP (CARE) to schedule an appointment.

Patients must review the Patient Information and Consent Form prior to their appointment and will be asked to sign the form on the day of their scan.

Patients will also be asked to review scan day instructions as well as be able to provide a medical history, list of medications and any special needs or requirements.

Patients also are asked to bring any recent test results (within last 6 months) and/or reports on their scan day. Any CD’s and/or Film will be returned to patients after their PET scan report has been issued.
